ACH Home Healthcare is looking for a confident and caring Travel Nurse to join our team. You must hold proven experience in clinical care, with a flexibility to work various shifts around your availability alongside established teams in community settings.
Responsibilities
- Delivering exceptional nursing care to patients across various locations.
- Coordinate with healthcare facilities to ensure continuity of care during patient transfer.
- Implement patient care plans and respond to changes in patients’ conditions.
- Provide health education and counselling to patients and their families.
- Ensure adherence to healthcare regulations and safety standards.
- Coordinating care plans with healthcare teams at multiple sites.
- Maintaining detailed and accurate patient records during travel and at each location.
- Educating patients and families on health maintenance and disease prevention during visits.
- Participating in continuous education to stay abreast of healthcare trends and best practices.
- Assisting with patient transportation as needed, ensuring safety and comfort.
- Providing post-travel follow-up care and assessments to ensure continuity of care.
